Timely Diver Injury Information

The accident details:

As required by Client, Diver was undertaking the lifting sling inspection at 5#-4#-3#-2# side of the new FSOU BTM, duration 75 min. According to the requirements of Client, Diver was conducting the first lifting of 3# lifting sling. When the first master link was lift to the required location, the diver began to pull the 4m×20Te SWL Round Sling through the master link. While he was hanging the two-headed sling eyes on the corresponding Hang-off hook (08:57), his right hand thumb and index finger were sandwiched between the sling and hang-off point, resulting crush injury. Diver reported to Supervisor immediately and supervisor arranged emergency recovery of the injured diver, at the same time Project Engineer, who was at the bridge, reported to Client Rep, Main Contractor Rep and DSV Master. At 08:59-09:10, Diving Physician conducted emergency medical treatment for the injured diver. Project Engineer informed Diving Contractor QHSE Manager, briefed about the injury and on-site measures taken. Client Rep informed onshore relative parties and activated emergency Medevac. There was helicopter available on the platform, and the injured diver was coordinated priority to be sent ashore for treatment. At 11:15, Diver and companion reached LF13-2 platform by basket and then took the flight back to shore and then reached hospital for further treatment.

Cause Analysis

Design Defect: the hang-off point is relatively short, approximately 13cm, while the diameter of the sling is 60mm, and the thickness of the two-headed sling eyes are about 12cm, likely to be detached. The moment the diver finished and left the hang-off point waiting for the lifting operation, the sling became unhooked, resulting repetition of the hanging for several times.

Operation Environment: The diver is working at surface, great swell made it very difficult for him to control himself. The diver had reported his bailout cylinder and helmet frequently hit the underwater structure.

Tools used: 4m×20Te SWL Round Sling, the sling is not banded, under the wave surge, the sling is very likely to be detached from the hook. The sling could have been banded in the middle and keep out the sling eyes at each end, easy for diver to operate and reduce the chance for unhook.

Human factor: After several repeating operations, diver may become irritable. And when another wave surges, worried about unhook again, he use incorrect gesture to grasp the sling, resulting the injury
