A widely known dive tour operator lost a dive boat to a fire this morning. The dive boat, the Laddawan 2, was in the news eight years ago (2002) when a member of the crew "had his stomach sliced open and lost severed limb while cleaning the propeller" near Koh Racha Yai.   4. 1997 0 0 Kurishio 1, Heavy Lift Barge, Bongkot field, Total, Japanese diver Thailand 80 Saturation 9 divers in sat, three man bell run, Barge hit broadside by large wave, roll sufficient to cause failure of the bolts connecting the crane boom to a “spindle� at the foot of the crane. The boom fell to the deck (just missing sat chambers but ripped the HRC off. HRC was not pressurized, doors closed) then fell overside taking the crushed HRC with it. Bell knocked sideways by falling jib, filled with water. The bellman emptied the bell and pulled one of the divers back in. When he tried to pull the second diver , all he got was the umbilical with bail out and mask still attached. The diver had bailed up to the surface. (The jib landed next to him, whipped by cables. (Heavy bruises on his back). He may have come across the HLC under the jib and assumed it was the bell flattened under the jib (similar colour and size as the bell), and having been trained in the Gulf where divers ditch rather than cut their umbilical, he ditched and swam to the surface where he was rescued alive. He was put in a DDC (on air) but died soon after. Fundamental cause was that the bolts on the crane had always been assumed to be in compression, not tension, and had never been inspected (specifically excluded by the certification body) . A number had previously completely failed due to corrosion. No allowance for wave motion. Reported by IMCA SF 03 98 (TC)

  6. 1989 11 4 Clegg Phil Thailand Oceaneering Diving Supervisor on the drillship 'Seacrest' that was capsised by typhoon Gay. Possibly a dive team in saturation, two divers possibly among the 6 survivors, remainder of the dive team believed to have perished with the rest of the crew.
  7. 1989 11 4 DB 15 Thailand McDermott After the typhoon (Gay), divers from the DB 15 put an airspread on a tug and headed towards the Seacrest location. On the way they rescued two fishermen clinging to a float. They arrived at night (after a helicopter had landed on the upturned hull and resuers had banged on the hull to try to locate survivors during daylight) and made an initial survey dive recovring one body caught in the rigging. The following day more bodies were recovered from the accommodation and more 5 from the bridge. Later, after autopsy, they were told that most of the dead recovered after the disaster had died from drowning from what they believed to be wind driven rain leading to the recommendation that rain b;ocking hoods be built into lifejackets. The surviving Indonesian diver's haliburton briefcase was recovered during the accommodation search. (Fraser diving's first saturation diving system broke off from the DB 15 during the storm (lost overside and sank) before anyone was in it. Longstreath.
  8. 1989 11 4 Drillship Seacrest Thailand Unocal 72 Ex 'Scan Queen', sister ship to the "Chancellorsville". Overturned in typhoon 'Gay' (Recorded as killing over 500 people and leaving 160,000 homeless in Thailand) in the Platong gas field with the loss of around 91 lives, 5 or six suvivors (very confused records). A helicopter landed two men on the upturned hull the morning after but no survivors - or indications of survivors (Tapping on the hull) were found. Oceaneering and McDermott divers recovered several bodies from inside the hull, (25 bodies recovered after the sinking in all). vessel was scuttled in 75metres water depth about three weeks later. Conflicting reports that there was an excessive amount of drill pipe on deck (therefore top heavy), that she was still connected to the welhead (though radio reports pieced together after the event indicate that the BOP was disconnected and the majority of pipe was left 'in hole' but may have left the well abandonment too late and not had time to de-anchor), or that modifications carried out in the Port of Satahip, Thailand, immediately before the storm were causal factors (addition of a heavy topdrive unit without the addition of compensating ballast and then sailing to site without carrying out inclination tests), also that the drill tower started tearing the deck plates as it rolled, peeling them back and allowing wave ingress

Authorities stated they would prosecute (occupational negligent manslaughter) the vessel operator, Tu Chuen-yi, for improper operation of the vessel and allowing the propeller to cut all four airlines leaving the divers at depth with no air supplies. None of the divers had licences, no standby or safety equipment.

  18. 1945 8 7 Zetterstrom Arne Sweden Navy 160 S/S Mixed Gas SWEDISH DIVER KILLED ATTEMPTING RECORD DESCENT. London, August 8 (AAP) – “Arne Zetterstrom (27), a Swedish navy diver, holder of the deep sea record of 364 feet, was suffocated when ascending from a dive in which he attempted to set a new record of 525 feet. Zetterstrom made his record last December, breathing a mixture of air and hydrogen, largely eliminating the narotic effect of nitrogen�. Reported in the Cairns Post, Qld. Arne Zetterström (1917 – 7 August 1945), researcher into the breathing mixture hydrox for the Swedish Navy. Zetterström first described the use of hydrogen as a breathing gas in 1943. From 1943 to 1944, a total of six ocean dives were made utilizing this mixture with the deepest to 160 meters (96% hydrogen and 4% oxygen). On 7 August 1945, Zetterström experienced technical problems diving from the HMS Belos. His support divers misread his signals and this was followed by a rapid ascent that resulted in severe decompression sickness and hypoxia.

The chief executive of MAG, said his team had cleared 1,500 explosive items in Sudan since last August and the work of the organisation had saved countless lives. D.K.P. Dassanayake. The 213-foot (65-meter) craft was empty of cargo, he said. "We suspect the blast was carried out by a suicide diver" he said. BBC News.

  24. 2012 3 1 Not Recorded Sergio Spain 40 SCUBA Aged 21, student at the Alicante Marine and Fisheries Institute, enrolled in a profesional diving course at the college, was on a training dive with a teacher and three other students to 40 metres, took out mouthpiece, swallowed water, panicked and made emergency ascent without stops. Given oxygen therapy and transferred ashore to a hyperbaric centre and treated for ommitted decompression. The teacher and one other student who assisted him to the surface undertook another dive to complete their decompression stops. The report commented that tests to assess students prior to being allowed to take the diving course (Swimming, breath holding for a minute and an 18 metre free dive) were discontinued 5 years ago and that has resulted "in a huge deficiency in the quality of the students" Reported by Informacion.es
  25. 2012 2 4 Rodriguez Miguel Angelo Spain Diveships 6 Aged 41, grout injection operation ('Emergency repairs') at the Queen Sofia pier in the port of Cadiz, call to the emergency services at 13:50 of a 'diver problem'. Initial reports indicated that he failed to surface, was recovered but did not respond to treatment, possible heart attack (pending autopsy).   29. 2011 5 9 Not Recorded Spain During an audit in Algeciras, a diving contractor discussed 5 incidents that had occurred over the previous 18 months with another diving contractor in the port. As the author commented, “it shows how much we are missing and how important it is to use competent diving contractors�:- 1 Dive on a tanker at night. Tender comes up on divers umbilical and recovers hat and bailout. Diver has jettisoned his equipment and swum to the shore (established later). Algerciras port closed in while search is carried out for diver. 2 Dive carried out following 30’ decompression profile. Diver had worked at 60’ for an hour and profile not adjusted for deeper depth. Diver collapses on deck after dive. Paralysis of lower limbs, tongue and visual disturbances. Type 2 decompression sickness, diver evacuated to chamber to Malaga hospital, 3 day treatment. 3 Diver carrying out propeller polish on Tanker – propeller starts while diver working and pushes him away – ripped shoulder and leg muscles. 4 Diver carrying out our hull scrub wearing Aga mask – tangle with hull scrub brushes/ loose rope and Aga locating straps (spider). Mask ripped from divers head.  Diver makes it to surface – partial drowning and hospitalized ( 3 months off work). 5 Three bailout delivery whips blow off the mask on the same day. PC

  31. 2011 3 23 Macias Javier Diaz Spain Macpherson Servicios Subaquacuaticos Aged 26, from El Puerta de Santa Maria (Cadiz), carrying out repair works on the (upstream) Analucian water board (Agencia Andaluza del Agua) Los Melonares dam gates near Castilblanco de Los Arroyos (North of Seville). One of a four man team. Emergency services called a 17:00 hours, body recovered downstream of the dam. No other details. Reported in Europa press/www.internetnews.tk.
  32. 2010 12 20 Bartolome 01 Juan Spain Albaladejo Fishfarm 0 SCUBA 34 Year old diver employed by a fish farm in San Pedro del Pinatar in Murcia, initially reported that he died after accidentally shooting himself in the chest with his own harpoon. It happened shortly before 9.30 on Monday morning when an emergency call came through that the diver was seriously injured when in the water and was being transferred by boat to the local port. All attempts to revive the man, both on board the boat and after reaching port, were however unsuccessful�. 'Accident in the worplace'. Reported by Typicallyspanish.com.   33. 2010 12 20 Bartolome 02 Juan Spain Albaladejo Fishfarm 0 SCUBA 34 Year old diver employed by a fish farm in San Pedro del Pinatar in Murcia, initially reported that he died after accidentally shooting himself in the chest with his own harpoon. See first report (Bartolome 01, above). 15 months after his death, following investigation "Jaula 23", two co-workers were detained in connection with firing the harpoon that killed Juan Bartolome. They were subsequently charged with negligent homicide, despite initially claiming that the accident was caused by the victim himself and reported as an accident in the workplace. The two witnesses said at first that he had died by accident when putting on his diving equipment prior to entering the water, but their statements contained contradictions and contrasted with evidence found in the boat and the diving equipment and investigators soon began to suspect the two of homicide by negligence. Forensic and reconstructive investigation concluded that the harpoon had been fired from a distance and under the surface of the sea, presumably by accident, by one of the diver's companions. The case against IHV, aged 58 from San Pedro del Pinatar and JFMN, aged 37, from Murcia is being held at the Magistrates' court in San Javier. Case ongoing (April 2012). Murciatoday.com
  34. 2010 3 7 Sanchez Lareo Anxo Miguel Spain 9 SCUBA Aged 40, asked to free the anchor of a sports boat by the owner 'who knew he was a diver' in the River Ares Estuary at 2 o'clock on a Sunday afternoon (Galicia), failed to surface.   35. 2008 11 17 B AC Spain Broth SA or Consomar SA 28 SCUBA Aged 50, head of a diving contractor from Valencia, Broth Sa, working (Inspection and filming) on an outfall 1 kilometre off the town of Moraira-Teulada (Spanish East coast, South of Valencia), failed to surface. Supervisor dived but could not find him. The next day, after a two hour search by a combined fire/civil guard diving team, his body was discovered trapped in fishing nets. He had ditched his cylinders/buoyancy vest in a last attempt to escape the nets when his air ran out. Reported by Levante-EMV.com

  38. 2008 5 23 Garcia Pol Carlos Spain Tinsa SA 30 S/S Air Aged 33, from Madrid, one of two diving supervisors in a 6 man team working on the dam on the river Agueda 5 km South of Cuidad Rodrigo (Salamanca, Western Spain) since February (4 months) on contract to OSEPSA - Obras y Servicios Publicos Sociedad Anonima - for CHD - Hydrographic Confederation of the river Duero . Ten minutes into a dive dredging with an airlift at 09:30, reported as stopped talking to topsides, stand-by diver located the diver at depth, unconcious, recovered to surface but failed to respond to CPR treatment.   43. 2008 0 0 Pol Carlos Spain 60 S/S Air ack hammer. He was on air as his breathing gas (at 60 m).  Due to the nature of the operation, the visibility was badly disturbed and to counteract this, the company had requested that the diver place a 10�-12� diameter flexi hose airlift in close proximity to his working position, to clear away suspended particles. This was secured by the diver using rope and was made hot at the surface.  There was no information as to whether the diver had a flow control valve at his side?  At some point the securing knot became detached, which caused the hose to drift out of position, coming into contact with the back of the divers head as he worked. The intense suction caused his band-mask to become dislodged and also trapped him at the base of the airlift, resulting in him drowning. Apparently several days prior to this particular incident there was a similar situation where the hose had dislodged and had trapped the working divers arm.  On that occasion topside managed to switch off the airlift and the diver managed to free himself - though he did require hospital treatment to his badly damaged arm. Furthers details may be coming. PC
  44. 2007 8 29 Rodriguez Joaquin Castelao Angel Spain Coral Diver 28 SCUBA Aged 74, known locally as 'the angel of coral' had spent over 20 years living in Alcudiq collecting coralharvesting coral ('Red Gold') from his boat 'Nemo' in the channel between Menorc and Mallorca, 17 miles north of the port of Alcudia, diving with his son, became ill during decompression stops. An SAR helicopter dropped a medic onboard who confirmed he had died. Had spent years criticising intensive fishing methoids (like beam trawling) which damage the seabed. The diver, aged 49 with 25 years of experience, worked at the factory in San Fernando, but participated in the work of putting ships into Cadiz dry dock (The vessel involved was the 'Rotterdam').   51. 2005 8 22 Not Recorded Spain German Navy Aged 24, diving off the German Minesweeper 'Homburg' at anchorage off Ageciras (Vessel was en route to Crete to join a NATO task force), described as 'on a routine diving exercise for training purposes in the Bay of Algeciras', diver appers to have suffered a decompression incident, initially treated onboard the minesweeper which then took him to Gibraltar (Because there are at least two Navy decompression chambers wheras there are none in Algeciras), medics from the Royal Naval Hospital Gibraltar met the vessel when it docked at the South Mole but in spite of intensive efforts including the use of a deibrillator, were unable to revive him. Doctors concluded that he had died whilst the ship as still at sea (Onboard the FGS Homburg and therefore on German soil) and the coroner released the body to the German authorities who flew it to Germany on a CL60 military aircraft for a post mortem. Contractor did not have the required permission from Maritime to carry out the work, no insurance, the diver had no medical, no in date SCUBA cylinder certificate, The ruling states that "The incident occurred about 11:00 pm on 6 December 2000, when the victim, who was turning 32, was unconscious with narcosis, anxiety, shortness of breath or over-exertion that could lead to carbon dioxide poisoning that caused death by cardiac arrest. These deficiencies pose serious violation of the applicable regulations posing a grave risk to workers leading to the death of the diver which would have been avoided had if the legally required security measures had been adopted by the accused�. Diver had no qualifications, and even if so would only have been certified to 25 metres under current legislation, no permit to dive from harbourmaster, no stand-by diver. The company manager was sentenced to a year and a half in prison, a fine of 1,080 euros, and ordered to compensate the family of the deceased with a total of 160,000 euros 5 years after the fatality. The boat's skipper, who was also charged has been acquitted as he only complied with the orders of his superior . The deceased was working on a fish farm in Zumaia when about 12.30 the crew found that the bow anchor was stuck on the bottom. When the diver was in the water, the boat manager twice gave the order to go hard forward to dislodge the anchor and the employee complied with this indication, when the diver was dragged into the propeller and sliced to death. The ruling states that the owner and manager of the company "was directly responsible to provide safe working conditions for their workers', despite which he allowed the work to be performed by a single diver, where the legislation requires two. The court also noted the propeller should not have been used with a surface umbilical diver in the water,' reckless manager’ allowing the maneuver. For this reason, it condemned the manager to one year in prison for a crime of homicide by negligence and six months for another crime against the rights of worker plus banned from managing a diving company three years, plus compensation to the parents of 14,100.

Team management was less than satisfactory. Lack of team co-ordination and the attitude of “I thought that was done” was in essence the main cause of the accident by explosive decompression, dual fatality. Bell seal was broken from TUP, system came to surface in seconds. Note, another, conflicting, report indicates there were 4 divers in the system and it was being used as a saturation spread rather than gas bounce drill support with three dead on arrival at surface and one died later, (He may have partially managed to close a door. Dive to release an AX ring. Freddie and Jimmy commenced a dive, Jimmy freaked, bell recovered and a third diver, Norman, was blown in. Jimmy calmed down and it was decided to send Jimmy and Norman down as (small) Freddie's suit was too big and he got very cold. Divers went through into the TUP and the O-ring blew and they went from 400' to surface in a few seconds. Freddie was injured (Cerebral and vestibular damage, many years treatment at Haslar and Newcastle University, still has life altering effects), but saved when hatch blew shut. Supervisor was also called Norman, may have committed suicide. No known reports - does anybody have information on this incident?

Divers had reported that as they salvaged the cargo they wre getting close to the remaining dynamite but it was supposed that the explosive had lost its power after so many weeks in the water. Three divers were down with a support boat carrying 7 men over the location, around 50 men were working on the nearby docks when the cargo exploded killing the divers and men on the boat, a further 30 were reported as seriously injured.   60. 1887 0 0 Tester Spain 49 Diver engaged to recover the last box of treasure (9 of 10 had been salvaged in 1885) from the Spanish steamer 'Alphonso XII' which sank of Point Gando, Grand Canaria in February 1885. Salvage team was using Heinke equipment and had been on site four months, the deceased had made about 60 dives but on his last dive stayed down some 36 minutes and after a 10 minute break went down again for a further 10 minutes. At first seemed well but then complained of pain in his left arm. Taken ashore and 'the usual treatment in case of pressure attacks was resorted to".   61. 2013 7 16 Robles Aller Fernando Spain Trabajos Especiales Maritimos 20m S/S Air Information received points to him cleaning rubbish/silt from outside dry dock gates at Armon Shipyard, Gijon, with a pump so they could open them to launch a couple of new boats, possibly a differential pressure incident. Spanish newspapers report that when his topside tender noticed a lack of bubbles and comms response he jumped in the water, in addition, there was also an Engineer supervisor and two labourers who helped in the rescue. Witnesses say that during CPR he bled from mouth and ears. An ambulance was called and attempted to resuscitate him. After an hour of attention by the EMTs, he was declared dead.

Taken out of the water unconscious, brought ashore and taken to hospital in Mazarron where he was in cardiac arrest, did not respond to treatment. Reported in la Voz de Almeria. No other details. The diver, whose body apparently washed up on the beach, was wearing a wet suit, goggles and two oxygen tanks and carrying a Czech-made submachine gun, a hand grenade, radio transmission gear and an underwater camera, the military said. Nearby, investigators found a cone-shaped aluminum submersible boat that could carry up to five commandos. ''Judging from the objects found, it has been proven that the dead diver was an armed infiltrator,'' said a Defense Ministry spokesman. A month ealier, a furor erupted when a South Korean fisherman caught a North Korean submarine in his drift net. The North Korean commandos on board apparently killed the crew and then themselves, but South Korean officials said that some of the commandos had recently been on South Korean soil. That incident aroused only limited outrage in the South, in part because the submarine was 12 miles offshore when it was caught in the net. North Korea said it had lost power and suggested that it had drifted south. A more serious episode occurred when a North Korean submarine ran aground on the South Korean coast in October 1996 and 19 crew members and commandos slipped ashore. Some 70,000 South Korean troops were deployed to hunt them down, and in the end all the North Koreans were killed or committed suicide except one who was captured and another who was never found. The body discovered this morning was found by a South Korean man walking along the beach near the city of Tonghae, 110 miles east of Seoul. Second diver to die on this operation (Lee Kwank-wook, 6th May), "A boom and a moan were heard, the diver was pulled to the surface, he was bleeding and unconscious, given CPR and transferred to hospital but declared dead there". Lee entered the water at around 1:50 pm to cut open the stern of the hull from the fourth deck, but as the operations drew to a close at around 2:20 p.m., a problem arose with the sound of crashing. At the crashing sound, two other divers at the scene pulled Lee out of the water but he was found to be unconscious with blood running from his nose and eyes, officials said. The chief neurosurgeon at Mokpo Hankook Hospital, said an X-ray and CT scan of Lee showed that both of the diver's lungs had been damaged by external injuries. "We suspect that he died from tension pneumothorax," the doctor said, referring to a condition in which the amount of air in the chest increases markedly. This was followed by erratic line signals 'similar to the emergency signal' and the deck crew began to recover the diver. "Ordinarily done by one man, this task necessitated the combined efforts of four, indicating that the diver was a deadweight and something was wrong. When the diver appeared at the surface, it was seen that the face glass of his diving suit, including the frame, had apparently been unscrewed and was missing. The water had rushed in and filled the suit and the diver was unconscious. A slight movement of the lips was the only sign of life. Artificial respiration was immediately applied but without success. Newspaper report dated 27th June regarding an inquest held in Simonstown into the death of a diver on the breakwater works. "Another diver named MacPhail was down at the same time, and whilst the two men were standing together MacPhail saw Kraming's helmet suddenly fly off. Kraming caught hold of MacPhail and pulled on the lifeline. Both men were drawn up to within 10 feet of the surface when the hauling stopped and MacPhail was forced to let go his hold with the result that Kraming fell to the bottom again and was drowned. It was conclusively proved that the helmet was properly ajusted when he left the boat to descend but there was no evidence to show how it became unfastened. The magistrate, in returning a verdict of 'death by drowning', commented somewhat severely on the methods prevailing at Sir John Jackson's works and expressed the opinion that an extra man should be furnished to each diver and that only reliable men should be employed on the work. It is said that such an occurrence is unparalleled in connection with diving operations". Straits Times.

"At 18h05, Sunday, 17th December, NSRI Durban duty crew were activated by the Transnet National Ports Authority (TNPA) following a request for assistance from Aquatech reporting a diver missing in the Port of Durban. We believe that the diver reportedly went into the water on a dive to relieve another diver who was already under a ship during dive maintenance operations under a ship berthed in Port. At 17h00, reportedly after it appeared that the diver had not arrived to relieve the diver who was already under water (under the ship), Aquatech divers initiated a search and at 17h15 raised the alarm calling TNPA Port Control. 'The diver went missing while working on the hull of a vessel on Sunday. He was not wearing a lifeline at the time‚ a device that could easily have identified his position in the murky water, his body was recovered on Thursday morning.“He was found in the area where he went missing‚ strong winds blew his partially decomposed body back to the wharf where he was found this morning. “The official cause of death still needs to be investigated‚ but the diver is believed to have died from drowning. “He was dressed in diving gear with his cylinders attached‚ but he was not wearing a mask at the time of recovery.” The police diver said many questions needed to be asked about why proper safety precautions hadn’t been followed. His badly decomposed body, missing off Western Singapore for two days, bobbed to the surface on Sunday evening; air tank taken by police for probe. In between running his own scuba diving firm, known as Jet Scuba, Mr Chia did freelance work for commercial diving companies. Relatives identified him through the tattoos on his body. The body of Mr Gerald Chia Jia Jie, 21, was found near the oil rig he had been working on, still clad in dark-blue work overalls and with his gas tank and face mask still in place. A Maritime and Port Authority of Singapore (MPA) boat, part of the search operations that had been under way for more than 50 hours, spotted the body at about 6pm. When Mr Chia went missing at around noon last Friday, he was on only his second assignment underwater for J Diving Services. He had started freelancing for the company only two days before that. The diver changed cylinders and returned to the water after a tea break but after a few minutes topsides realised there were no bubbles coming up, tugged on his lifeline, no response. Diver pulled to the surface, unconscious, transferred to hospital, declared dead. At the inquest, Singapore Oxygen Air Limited (SOXAL) stated they had supplied 25 cylinders of compressed air for industrial use (Light grey) and one cylinder of Nitrogen (Light grey with a black top) a few months earlier and that each cylinder also had a label stating the contents attached before they left the factory. They also added that cylinders with light grey bodies and black and white quarters at the top contained air for medical uses. The storeman who issued the tanks on the day had not noticed any gas contents labels on the cylinders but recalled that the cylinders were gray and at least one had black and white quarters. Additionally he said that he was not aware of which gases were in cylinders with different markings nor what colour a nitrogen cylinder should be as he was not in charge of sorting them. Investigators did not find any label on the Nitogen cylinder used by the diver. The Coroner recorded an open verdict and criticised the parties involved for their apparent lack of safety consciousness. Straits Times

They attempted artificial respiration which was further continued by arriving doctors, but no response. The investigation concluded that the air line had been caught and severed by the lifting gear due to the strong tide (slack air line) and that the surface crew probably did not understand the diver's signals. Cause of death was given as asphyxia due to suffocation. The coroner observed that "the deceased was dead when brought to the surface". Straits Times.

Mohamed Firdaus was working as a commercial diver for Marine Diving & Engineering (MDE), which specialises in underwater services including marine and underwater construction. On the day of the accident, Firdaus was working on a project developed by Vopak Terminals Singapore on Pulau Sebarok’s Jetty 2. The project's occupier is HSL Constructor, which had engaged MDE for the project. "Two divers were in the water, installing brackets onto a concrete pile, when one of the workers was struck by a concrete pile," said the Ministry of Manpower. Firdaus was unconscious when taken to National University Hospital, where he died from his injuries, added the police, who received a call for assistance at 5.13pm. In an email to Channel NewsAsia, Vopak Terminals confirmed that a fatal accident happened at its terminal in the port. "We are deeply saddened by this accident. Longstreath. Leak investigation on a live 12" water injection pipeline at between 800 and 900 psi. "The diver successfully attached the marker buoy to the pipeline but it became fouled under the vessel. In order to free the buoy the diver was asked to move it to the far side of the leak. At the time of the dive, visibility was reduced as it was approaching sunset; the diver used his hat-mounted light to navigate. In addition, the tremendous noise of the leak on the live pipeline severely hampered voice communications between the diver and diving supervisor. Once the diver had detached the marker buoy he started to move along the seabed beside the pipeline. He was on the same side as the leak. The diver appeared to look down at some debris and, as he stepped over this material, he was struck by a jet of the high pressure water coming from the damaged pipeline. All communication with the diver was instantly lost. The standby diver was deployed immediately using the second diving basket. Within 5 minutes he had located the stricken diver who was lying on the seabed. The dive helmet (KM37) was no longer on the diver's head but was lying beside him with the neck dam still attached. Apparently entered the wreck without using a lifeline (Ispas worked for the Naval emergency services but worked also for the diving contractor, unclear if he was a qualified commercial diver, Zarrafu was a full time commercial diver. Proecution a year later claimed no lifelines, insufficient number in the dive team, improper equipment (no communications, use of sports diving equipment for commercial purposes), misconduct. After 5 months of salvage operations the vessel was refloated in early December but then sank again two days later. The owner of the salvage company said that they had suspended ongoing salvage operations the previous week (waiting on a salvage vessel to lift some components prior to bringing the vessel ashore) and left the two divers guarding the wreck. They were reported missing on Wednesday the 13th, Alger Sumaylo's body was located inside the wreck at low rtide (partially submerged but trapped under the hull) by other salvage contractor divers on the morning of the 14th, they called the coastguard who sent a team who recovered both bodies later that day. Initial reports said that when recovered, both divers "were in complete diving gear" and it was reported that the bodies were already decomposing. The owner surmised that the two divers "had decided to dive to retrieve scrap meta whilst everybody was away". Foul play was ruled out as the divers belongings were intact. Later reports said that the coastguard had asked Soco (Scene Of Crime Operations) to check where the bodies were located as they were not discounting foul play. Apparently he 'insisted on fixing a a leak in the cooling water pump area himself' 'with the pump running'. Quote from colleague:- 'When we tested the pump (with the diver still in the water...) he was probably taken by the vacuum to the water pond which is really deep, he must have lost his oxygen in his tank and drowned'. After 15 minutes and he had not surfaced, the crew started to get worried and stopped the pump to carry out a search. Body recovered and taken to hospital but declared dead on arrival. ABS CBN News

Volunteered to dive to about 221 feet, as another PCG colleague was unavailable. On the first of three scheduled dives for the day, Bonifacio, along with his dive buddy, retrieved a woman’s body. On their second attempt, the two reached the ship’s wreckage again. On their ascent, however, diver felt Bonifacio shake his hand at about 170 feet deep, a signal that he was not feeling well. Bonifacio reportedly increased his ascension speed, which was against basic diving safety rules. “He wanted to speed up his ascent, which would put him more in a very compromising situation. He was held back although his buddy assisted him in breathing in air)," SOG diving team head Lt. Commander said in a GMA News’ 24 Oras report. At around 140 feet, Bonifacio lost consciousness. He was resurfaced and brought to a decompression chamber for first aid. Inside the chamber, things went well as Bonifacio regained consciousness and was reportedly still able to follow orders. Two hours later, Bonifacio succumbed to cardiac arrest and was pronounced dead on arrival at the Jose Reyes Memorial Medical Center. In an ABS-CBN newscast, Bonifacio’s grieving wife rues the death of her husband, who had been in the service for 18 years and was already looking forward to retirement. “I wanted to stop him, as he was getting weak physically," the wife was quoted as saying. Bonifacio had been an integral member of the SOG diving team, had been trained by the Philippine Navy and the Japan International Cooperation Agency. He participated in the relief operation for the typhoons Ondoy and Pepeng, which hit the country in 2009. He was also part of the rescue operation for MV Princess of Stars, which sank in Romblon in 2008, where 300 of the 800 passengers remain missing. “We checked the equipment. It’s not the equipment," but a PCG spokesperson admitted in the newscast that government divers do not have the required gas mixture for deep-sea diving. GMA News

The newly designated police chief reported that Peter Jao, a diving expert of the CJMS Diving Services, did not reach the hospital alive. He suffered broken ribs and skull. The police report stated that the cause of the falling incident was due to a foul odor that the victim inhaled while climbing the ladder on his way out of the chlorination tank. As Jao fell into the cemented flooring of the chlorination tank, his companion identified as Nenito Quintana, also a diver, went down the tank and tried to rescue him. Quintana was assisted by other employees using a crane. As Quintana was climbing the ladder on his way out, he also fell. Rescuers rushed Quintana into the St. Michael Hospital in Orion town. Paraphrased from the book by Wilburn Cobb. “A Palawan island tribe on a conch fishing expedition, realised one of the group, Etem, was missing. Suspecting a giant octopus, they unsheathed their knives and dove down in search of their missing comrade. On the fourth dive they found Etem already dead, his left hand trapped between the shells of a giant Tridacna clam. With the aid of ropes, the men hoisted their dead comrade and his deep-sea murderer into one of the canoes. As the death needed to be fully explained to the authorities, they took the boy just as they had found him with his left hand still in the grip of the giant shell to chief who acted as the local notary public. The boy was buried, but three days later as the chief watched his men remove the meat from the shell, he saw an enormous pearl (It weighs over 14 lbs). Two years later it was given to Cobb as a gift after he saved the Chief’s son from dying of malaria. Eleazar Viernes, 63, a diver from Olongapo City, died while working underwater with another worker to install a dummy gate for replacement of valve of its Pacific Dock. Viernes, however, was reported by another worker to have suffered difficulty in breathing underwater. He died an hour later at the hospital.

(KFVS) - A diver died while working on the Olmsted Lock and Dam project on Saturday, June 29. According to spokesperson with Moran Environmental Recovery, LLC, a commercial diver with Global Diving and Salvage died while working on the Olmsted Lock and Dam project. Terrence Linston reportedly had a incident while he was under water and did not survive. He had been employed with Globan since 2017 and had been working on the Olmsted project for two years. Global is contracted by the U.S. Army Corps of Engineers. The U.S. Army Corps of Engineers released the following statement on Wednesday, July 3: “Our thoughts and prayers are extended to the family members during this difficult time. We are closely coordinating with all parties as the cause of the incident is still under investigation.” - Army Corps of Engineers, Louisville District According to the Moran Environmental Recovery spokesperson, they are cooperating with authorities and the investigation is ongoing. In May 2019, a massive crane specifically built to assemble, move and lift sections of the dam was demolished. In August 2018, a ribbon cutting was held for the Olmsted Lock and Dam project. The project, which replaced 1920’s-era infrastructure, hoped to make an easier navigation of commerce on the Ohio River.   238. 2007 8 12 Not Recorded Norway Saipem On 12 August a 48-year-old man died in an accident during work (lifting operation) on board the crane vessel Saipem 7000 near the Gullfaks field in the Norwegian sector of the North Sea. At around the 1000hrs the man fell overboard and was found dead shortly afterwards following a rescue operation. The deceased was an employee of Saipem, who during the weekend of 12/13 August has been working for Statoil installing a subsea separator at the Tordis field. The cause of the accident is being investigated (Actually found one and a half hours later on the seabed by the ROV) first offshore Norwegian fatality since 2002.
