Williams, under loan from the Australian Government, in raising the British India liner ‘Santhia’ which was on her side in the Hoogli River. ‘Santhia’ has been brought to an upright position ready for refloating. London, August 8 (AAP) – “Arne Zetterstrom (27), a Swedish navy diver, holder of the deep sea record of 364 feet, was suffocated when ascending from a dive in which he attempted to set a new record of 525 feet. Zetterstrom made his record last December, breathing a mixture of air and hydrogen, largely eliminating the narotic effect of nitrogen�. Reported in the Cairns Post, Qld. Arne Zetterström (1917 – 7 August 1945), researcher into the breathing mixture hydrox for the Swedish Navy. Zetterström first described the use of hydrogen as a breathing gas in 1943. From 1943 to 1944, a total of six ocean dives were made utilizing this mixture with the deepest to 160 meters (96% hydrogen and 4% oxygen). On 7 August 1945, Zetterström experienced technical problems diving from the HMS Belos. His support divers misread his signals and this was followed by a rapid ascent that resulted in severe decompression sickness and hypoxia.

Some time ago when the battle ship Dreadnought was there, one of her seamen divers went below to clear her propeller of some flotsam but failed to come to the surface again. Signals by telephone and lifeline were sent to him without avail, and when some brushes and other things came to the surface it was feared that the man was dead. Night training dive for Israeli Naval Commando, simulation of combat dive in enemy port. “The dive was a group exercise, carried out in pairs, simulating a combat dive at an enemy port. The divers had completed similar exercises dozens of times since the beginning of the course. The exercise was supervised by Navy commandos on boats as well as on the beach, and an ambulance and a medic were standing by. The dive was supposed to last two or three hours, and each diver was to take turns being the lead diver. Some 90 minutes after the dive began (Around 02:00), when Azoulay became lead diver, he stopped responding to the routine once-a-minute check. When his partner realized that he was not responding, he carried him to the surface and fired a flare gun to mark their location. Within seconds, a boat arrived and Azoulay was given CPR. He was pronounced dead on shore 40 minutes later.' investigation by Israeli Defence Force concluded that he died from oxygen poisoning. Magen David Adom paramedics evacuated him to Rambam Hospital in the city, where he was pronounced dead. As far as is known, he was injured while operating underwater pumping equipment. The Hof police station reported: "At the center of 100, a report was received about a resident of the Galilee who was injured - apparently during underwater work when he operated equipment that he had in his possession. Researchers from the Ministry of Economy and the Ministry of Transport were summoned. " Ronen Keren, CEO of Nahshon Underwater Works, said he still did not know the exact details of the incident under investigation: "What can be said at the moment is condolences to the family of the worker who was killed," he said. Mohammed Nasrallah Hamdan, 33, from Majd al-Krum in the Galilee, was survived by a wife and two young children, aged 6 and 4. The prosecution arose from a spot inspection by HSE at underwater maintenance work on moorings in the St Mawes area of the Falmouth estuary during which an inspector observed breaches of the regulations. ‘The dive was being carried out without essential equipment, using only one cylinder of breathing gas, when a secondary supply is mandatory, and the diver only had a mouthpiece demand valve when a full face mask was required. There was no standby diver, a lifeline or a line for voice communications with the diver who was only qualified to carry out recreational dives, not commercial ones. The diver did not possess a valid certificate to prove his medical fitness to dive. Demobilising an air spread, attempting to remove an air receiver tank from the deck of the barge with a 150T crane. The signalman signalled the crane operator to lift the vessel. As he was lifting the vessel it did not move or rise. The signalman instructed the crane operator to stop lifting (but not slack tension) and the Deck Supervisor started checking on what was the problem. As he was checking around the air receiverl it suddenly popped loose and struck the Deck Supervisor in the head causing major injuries to the back of his head.

Further information has been received by the Inspector of Police, Cairns, regarding the recent death of a Japanese diver, aged 20 years, off Thursday Island. The deceased was engaged by the Morley Pearling Co. of Thursday Island, and was diver and captain of a ketch named "Dart." He was at work in six fathoms of water when the spring of the pump burst, and he was immediately hauled to the surface. When his head-gear was removed he was found to be dead. The scene of the fatality was at the fishing grounds, 60 miles from Thursday Island, known as Badu. Deceased was-taken to Thursday Island, where death was declared to be due to suffocation while diving in apparatus. Reported in the Cairns post, Qld.

They were a Japanese diver, Ichitaro Miyao, 35, and a Mapoon native, Sammy Myquick, 17. The lugger was working at Warrior island with the diver down five fathoms. When no signals had been received for some time the captain went down and found Miyao dead in his driving dress. As the lugger was returning to Thursday Island Myquick was found dead in the hold.   26. 2018 5 5 Seet Choon Heng (Jake) Singapore Mola Subsea Services SS/Air The body of 33-year-old Jake Seet Choon Heng, who went missing off the waters of Sentosa on Saturday (5 May), has been found. The police were alerted to a body found floating in the sea off Sentosa at 6.36pm on Monday. The body was that of a 33-year-old man who was reported missing on Saturday. The police are investigating the unnatural death. Seet, a 33-year-old commercial diver, went missing while conducting underwater operations for the vessel Jork at the Western Anchorage near Sentosa on Saturday. He is understood to be employed by Mola Subsea Services, which provides commercial diving services for vessels at major ports and anchorages within Singapore. On Monday, his sister Jacqueline Seet made an urgent appeal for “all parties to do their utmost” to bring her brother home. In a media statement, she added that neither the authorities nor her brother’s employer had promptly informed their family about his disappearance. Yahoo News Singapore has reached out to Seet’s employer for comments. Jake Seet leaves behind two sons aged six and three. According to his sister, his wife is due to give birth to their third child, a daughter, next month. Jasper Lei, 35, associate marketing director at Propnex Realty and a former colleague of Seet’s, described him as a “very fit and experienced diver” with more than 10 years of experience. While Lei expressed deep “shock” over Seet’s passing, the former commercial diver also noted that safety on the job could be compromised at times, due to the urgency of the task at hand and “limited manpower issues”. Lei and a friend started raising funds for Seet’s family on Monday night and have collected about $3,200 so far. “Jake came from a humble family, his wife is definitely devastated by the loss,” Lei added. “With two kids plus an unborn child, the future is really challenging for them.” This story was updated to include quotes from Jasper Lei and other information. Storms prevented the new rig from being moved to its first drilling location and the rig was anchored in 342 feet of water about 100 miles east of Shetland. Between 29 Dec 1973 and 01 Jan 1974, the rig suffered progressive structural damage resulting from the storm conditions leading to the evacuation by helicopter of 38 of the 56 crew at around 1800 hours on 01 Jan 1974. The remaining 18 crew were then evacuated by 2300 hours on 01 Jan 1974. The crew were transferred to the nearby Transworld 61 and Transworld 58 rigs. Four tugs were on stand-by to tow the Transocean 3 to Norway for repairs, but continued bad weather sank the rig in the early hours of 02 Jan 1974 before this could be performed. HSE documents state that a jackable leg broke away from the machinery house, leading to the subsequent capsize and later sinking of the vessel.   29. 1979 5 10 Rig 'Ranger1' USA Mitchell Energy 3 leg Jack Up built 1968 drilling block 189L. Between 1500 and 1800 hours in the afternoon experienced a violent shudder, with personnel reporting it as a violent shaking or a vertical fall of up to 30cm. Up to an hour was spent trying to locate the cause of the movement without success. Work aboard the rig then continued, including the offloading of supplies from the Delta Seahorse supply boat. At about 2230, prior to a shift change, the Ranger I collapsed into the sea. The stern leg, below the accommodation block, broke near the connection to the support mat causing the upper hull to fall stern-first and strike the Delta Seahorse. The bow legs supported the upper hull for a short time, then collapsed, allowing the upper hull to fall to the sea. The upper hull then separated from the legs, drifted to the west and sank over the course of the following day. Prior to the initial collapse, most of the crew were in the living quarters, with 13 men in the galley. The survivors reported experiencing a rapid fall followed by sudden jolt as the upper hull struck the water. The lower floor of the accommodation unit flooded instantly to a depth of 2-3 feet. The men escaped either through the galley's port door or through galley windows taking seat cushions or life jackets for floatation. Three patterns developed: one group gathered at the helideck; a second group swam to the Delta Seahorse whilst a third jumped overboard to get away from the upper hull before it sank. The Delta Seahorse, alongside the Ranger I, signaled a MAYDAY and picked up four crew from the water, going on to co-ordinate further rescue operations. The Miss Angela, busy towing another rig at the time of the collapse, was also dispatched to aid the rescue and picked up a further 14 men. Coast Guard helicopters recovered another four men from the water. Eight men lost their lives in the accident, with many of the survivors suffering serious injuries. The remains of the rig were later salvaged and examined by the U.S. Coast Guard. The investigating board concluded that the Ranger I had collapsed as a result of an existing fatigue crack in the stern leg, near the connection to the mat. At around 1700 hours on 10 May 1979, the crack had rapidly propagated around the leg, causing the leg to break and the rig to shudder. Over the following hours, a combination of dynamic and static loading dislodged the broken leg and caused the rig to collapse.

Initial report stated "undertaking pier side diving training at the Alpha Pier of Guam Naval Base" (Most likely actually at 'Apra' pier - the vessel's home port is Apra Harbour, Guam, TC). Described as "not diving, but observing other divers from the surface in snorkeling like role", not clear if he had diving gear on. "When the other divers surfaced, they noticed Dotzler was missing and located him at the bottom of the harbor, he was pulled from the water unconscious and later pronounced dead at Guam's Naval hospital. Fleet Command in Constanta said they could not yet provide details of the circumstances in which he was injured. He said that he was in training at sea and the ship returned to port. The injured diver brought to shore and taken by ambulance to Hospital but could not be saved. Doctors said the diver has died after a cardio-respiratory arrest and that he had a severe acute head trauma. 

Health professionals say that the first information received shows that the diver was submerged in water and was injured in collision between two boats. Reported in Realitatea.net. A military diver aged 36 died and three others were injured yesterday morning during a training exercise at sea outside the port of Constanta. It seems that the tragedy took place after an inflatable boat did not respond to commands and simply passed over a similar one, in which there were many divers.   35. 2000 11 23 Moscow USSR Military An elite, specially-trained team of combat divers guards the Kremlin against potential attackers trying to gain access to the Kremlin through the sewers underwater from the River Moskva and the underground network of sewers. But any would-be transgressors would be met by sinister-looking combat divers - known as strategic-purpose divers. The first units of combat divers were set up in the 1960s to combat underwater saboteurs. They now form part of the Presidential Bodyguard Service. The divers all have officer's rank and get free flats in Moscow. As well as patrolling the sewers, they also inspect the River Moskva around the Kremlin, protect all the presidential residences from offshore and accompany the president when he goes to the Black Sea resort of Sochi. A special underwater pistol was designed as a non-automatic four-barrel gun loaded in the same way as a hunter's rifle, by opening the breach. The bullets look strange too. A bullet is actually a long needle or a "nail" as the divers call it. The nails can kill at a distance of six to 17 metres, depending on the depth. The divers say that underwater fighting with knives only exists in films. A basic principle of underwater combat is that whoever attacks first, wins. Even the slightest wound could be lethal underwater because water pressure leads to massive loss of blood which renders the diver useless in seconds.   36. 1998 7 13 Not Recorded South Korea Military South Korean military commanders put troops on the country's eastern coast on alert and declared a curfew in the area after finding the body of a diver who they said was a North Korean commando. The diver, whose body apparently washed up on the beach, was wearing a wet suit, goggles and two oxygen tanks and carrying a Czech-made submachine gun, a hand grenade, radio transmission gear and an underwater camera, the military said. Nearby, investigators found a cone-shaped aluminum submersible boat that could carry up to five commandos. ''Judging from the objects found, it has been proven that the dead diver was an armed infiltrator,'' said a Defense Ministry spokesman. A month ealier, a furor erupted when a South Korean fisherman caught a North Korean submarine in his drift net. The North Korean commandos on board apparently killed the crew and then themselves, but South Korean officials said that some of the commandos had recently been on South Korean soil. That incident aroused only limited outrage in the South, in part because the submarine was 12 miles offshore when it was caught in the net. North Korea said it had lost power and suggested that it had drifted south. A more serious episode occurred when a North Korean submarine ran aground on the South Korean coast in October 1996 and 19 crew members and commandos slipped ashore. Some 70,000 South Korean troops were deployed to hunt them down, and in the end all the North Koreans were killed or committed suicide except one who was captured and another who was never found. The body discovered this morning was found by a South Korean man walking along the beach near the city of Tonghae, 110 miles east of Seoul. The team also carried out salvage work and trawler searches to protect and secure South Vietnamese ports from sabotage. Over this period there were 7573 ship searches, 153 major diving tasks, 78 explosive devices removed from ships, 352 tons heavy ordnance destroyed, 42,000 items of unsafe ammunition destroyed, 68 special operations including canal barricades, search and destroy missions plus reconnaissance and ambush missions in three fire zones. Casualties were one clearance diver killed and one clearance diver wounded on active service.

Trainee diver's lifeline 'appeared to be fouled and broke', Lt Barron was the standby and was sent into assist, gave a thumbs up and submerged. There was a terrific jerk on his lifeline and then his line broke too. It was later found that both men had been sucked through an open penstock (equalisation pipe between two docks). Diving instructor admitted no diving flag was being flown, civilian dockyard foreman in charge of the sluices stated that he was not aware of diving operations being conducted in the main basin and would have considered the dive site as hazardous because of the penstock (Diving in that area had been stopped two years earlier because of the penstock). No formal communcation link between divers and dock personnel. Following the incident Admiralty orders were put in place covering operation of diving in the docks and civilians in the dockyard also had definite instructions

In 2007, Eduard Koltsov, retired Russian diver claimed to have killed him and cut his head off after he caught Crabb placing a mine on the hull. MOD admitted previous underwater surveys by RN divers on visiting Russian vessels. Other memos released in 2007 indicate that MI6 recruited Crabb for a separate mission and that he was not alone.

He recovered and returned to the chamber, but passed out. Water had got inside his breathing circuit/helmet and got into contact with his air purifier, and he had "burns about the neck". Treated at Bethesda Naval Hospital but died. "Officers said the lye burns caused death" (Sodium Hydroxide/caustic soda, burns to the throat). The Milwaukee Journal.

Ballard wore a specially adapted diving suit. His object was to enable naval technicians to perfect a diving suit in which rescuers can work at crash-dive depth of submarines". Curiously, this record was also reported on the 10th of March 1950 in the Horsham Times (Victoria) with a photograph of Bollard being congratulated by fellow Royal Navy diver W. Soper who had also broken the previous record with a dive to a depth of 454 feet.

His citation reads:- "For conspicuous gallantry and intrepidity at the risk of his life above and beyond the call of duty as a diver engaged in rescue operations at West Loch, Pearl Harbor, 17 February 1945. Aware of the danger when 2 fellow divers were hopelessly trapped in a cave-in of steel wreckage while tunneling with jet nozzles under an LST sunk in 40 feet (12 m) of water and 20 feet (6.1 m) of mud. Hammerberg unhesitatingly went overboard in a valiant attempt to effect their rescue despite the certain hazard of additional cave-ins and the risk of fouling his lifeline on jagged pieces of steel imbedded in the shifting mud. Washing a passage through the original excavation, he reached the first of the trapped men, freed him from the wreckage and, working desperately in pitch-black darkness, finally effected his release from fouled lines, thereby enabling him to reach the surface. Wearied but undaunted after several hours of arduous labor, Hammerberg resolved to continue his struggle to wash through the oozing submarine, subterranean mud in a determined effort to save the second diver. Venturing still farther under the buried hulk, he held tenaciously to his purpose, reaching a place immediately above the other man just as another cave-in occurred and a heavy piece of steel pinned him crosswise over his shipmate in a position which protected the man beneath from further injury while placing the full brunt of terrific pressure on himself. Although he succumbed in agony 18 hours after he had gone to the aid of his fellow divers, Hammerberg, by his cool judgment, unfaltering professional skill and consistent disregard of all personal danger in the face of tremendous odds, had contributed effectively to the saving of his 2 comrades.   95. 1944 6 6 Not Recorded France Military From the Obituary of Lt Cmdr Robbie Robinson:- “As a member of a landing craft obstruction clearance unit, Petty Officer Robinson had the task of opening a path through the booby-trapped obstacles on the beaches below the high water mark. This meant defusing improvised and unfamiliar deadly explosives on the shore and underwater while being sniped at and sprayed with machine-gun fire. The unexpectedly heavy surf made his task all the more tiring, but after opening an initial path his team had cleared a gap in the enemy defences 1,000yds by 400yds by the end of the first day. Eventually, he helped to clear more than 2,500 obstacles. Two other naval divers were killed during these operations and 10 injured. Robinson himself was knocked out when six feet underwater by an explosion that left him paralysed for several hours. A Royal Engineer working close by was killed outright, but Robinson was saved by a specially designed Kapok jacket under his diving suit. Although he was left with back trouble ever after, he returned to work on the beaches and harbour of Cherbourg a couple of days later�. Reported in the Telegraph (UK).

Two divers died on operations cutting into the wreck of the Arizona (referred to as 'due to air pockets' but probably underwater oxy arc explosions, TC) and after consultation with relatives it was decided not to attempt to recover any more bodies 'Enough were dead, it wasn't right to risk any more lives' and she remains a war grave. They removed parts of the superstructure and some guns, but the hulk still lies where she sank, atop the wreck is a white marble monument (Fund raising was pushed by Elvis Presley) The Arizona monument is widely reported, the deaths of the two divers was reported in the Deseret News in 1963.   124. 1939 6 1 HMS Thetis UK Military 45 Submarine The “Thetis� arrived in Liverpool in May 1939, sea trials started on 1st June in Liverpool Bay with a tug in attendance. 59 crew, 2 caterers plus an additional 44 men aboard, bringing the complement up to 103. The boat began a dive but refused to go down. After other attempts and making some adjustments, with no other vessels in sight, the Captain ordered some of the Torpedo Tubes to be flooded. This action was not entirely successful, and suspecting that one tube had failed to flood, a valve on the tube hatch was opened to check for water. Nothing came out, and assuming the outer tube door had jammed, the hatch was opened. The valve was faulty, it was fouled with paint, and rush of water began to flood the compartment. The Torpedo room crew retreated to the next compartment and closed the hatch but it failed to work and the men from both sections retreated into the next chamber and the hatch closed. With two bow sections flooded and at a steep angle, they blew all the boats ballast tanks. The submarines bow struck the bottom, 150 feet below, leaving 18 feet of the stern above the surface. The nearest escape hatch, was just 20 feet below the surface, but following standard practice, it was not used until the vessel had been located. But the surface tug had lost track of the Submarine and it took over 17 hours for the Destroyer " Brazen " to find them, but with an emergency supply of air for just 24 hours, the extra men on board used it up at twice the rate. With rescue vessels on hand, 8 men evacuated (four drowned) before the hatches failed and the stern flooded forcing the tugs to cast off. Thetis sank with her remaining 95 men trapped onboard. Bad weather followed, making any attempt at salvage impossible. Salvage stated on 24th August (a navy diver, PO Henry Perdue, died during the operation 24/8/1939). The Submarine was recovered and refitted as the “Thunderbolt�, but suffered heavy damage during a depth charge attack in the Mediterranean and sank in over 3,000 feet of water. She was refloated the same year and returned to service), reached the seabed and the repeatedly signalled for less air, then lost communications. Surface crew tried to pull him up but his lines were fouled (Although only diving in 11 fathoms they had paid out 200' of hose because of high currents and wanting to give the diver freedom to move about). They then manouvred the vessel and freed the lines bringing the diver to the surface 15 minutes later, but he was dead. Believed that his lines caught round the stump of an old mast and in an attempt to free himself had dropped his weights at which time he had floated towards the surface then held upside down by his lines and suffocated from lost air supply. "If he had waited perfectly quiet in deep water he could have been saved". They had a second diving dress on board but only had 50' of hose left so could not reach the diver. Inquest verdict was 'death from misadventure' with the coroner commenting that the diver's shipmates appeared to have done all that was possible in the circumstances. Four men were killed and several seriously injured at Fort Independence, on Castle Island, in Boston Harbor this afternoon, by the explosion of a mine which had been removed from the channel. Those killed were Sergt. Morris McGrath, Hiram Vaughn, Private Brennan, (a diver.) and a man named Ryan. The accident occurred about 2 o’clock. Vaughn, Brennan, and Ryan had placed the mine on a cart, and started to take it to the place where the other ammunition was stored, when from no apparent cause the mine burst. The horse and cart and the three men were blown to fragments. Sergt. McGrath was standing fully twenty feet away. He was instantly killed by the concussion. His body does not bear a mark of any sort. As soon as possible after the explosion, a careful search was begun by Lieut. Raymond, who had charge of removing the mines, but not a fragment of the three men or their clothing could be found. Lieut. Raymond says that he cannot explain how the explosion occurred. He says that since the order was issued from Washington hundreds of mines have been handled in precisely the same way as was this one and transported over the same road, and no harm had resulted. Reported by Scheepvaartnieuws.The following details have been paraphrased from OGP safety Alert 248. “Routine surface supplied diving operation to clean marine growth from a vessel hull using a hull cleaning device with rotating brushes (a brush cart that required a diver to guide and operate it). The brush cart contained diver operated tooling actuated by an air driven piston. Standard company practice was to use the suit inflation take off from the diving mask for this purpose. Prior to this dive the diving mask was changed to a model that did not have a suit inflation take off capability. A decision was made to connect the diver's emergency gas supply line (Bailout) directly to the brush cart tooling. This resulted in the diver having no personal emergency gas supply. During the dive the diver's umbilical was caught in the wheels of the brush cart and the diver experienced a reduction in breathing gas. The diver, having no emergency gas supply, removed his mask, immediately losing communication to the surface team and was observed in distress at the surface. The surface team was composed of a radio operator (a diver) and a tender for the diver (a non diver) and a diving supervisor - who was not on the site at this time. An attempt was made to pull the diver back using the diver's umbilical, this was ineffective. The radio operator, who was also the standby diver, jumped into the water without diving equipment and attempted to rescue the diver. The brush cart was negatively buoyant and the rescue diver was unable to prevent it sinking with the diver attached. The standby diver returned to the vessel and dressed into the standby diver's equipment (SCUBA) and recovered the body of the diver from the seabed. What Went Wrong? i) Risks with the work equipment were not identified and addressed, ii) Critical Safety equipment was misused to achieve the work task, iii) Personnel levels were inadequate, iv) No onsite supervision, v) Inadequate Emergency recovery equipment. Corrective Actions and Recommendations: i) Utilise the OGP RP 411, Diving Recommended Practice as the baseline standard for diving, ii) Verify contractors are in compliance, iii) Establish that adequate risk assessments have been carried out, iv) Ensure minimum personnel levels for diving are 5 (One supervisor who cannot leave the dive site, a diver, a diver's tender, a standby diver and standby diver's tender), all personnel should be diver qualified and competent, v) Verify emergency breathing supply equipment is of suitable volume and immediately available, vi) Confirm that any use of the divers breathing gas supply for tooling power is unacceptable, vii) Ensure the standby diver's equipment is the same as the divers, viii) Verify that while any diver is in the water, the standby diver is dressed and in immediate readiness to carry out a rescue, ix) Ensure emergency recovery equipment and procedures are adequate to achieve recovery, x) Verify emergency drills have been carried out to test the emergency recovery procedures with the diving team. It was about four minutes from the time the pipe burst until on diver was on surface, bleeding from his mouth and eats, possibly "diver was little bit alive." One witness stated that he had complained that the length of piping which burst was not good. The owner denied this (the same length of pipe had been used on the previous tide to 19 fathoms and stated that he had 22 years practical experience of pearling, and attributed present mishap to a flaw in the piping. “The life of piping with steel wires was about 4 years, and this piping had only been in use two and a half seasons. A diver who knew his business would at once close the air escape valve in his helmet. If that were done there was, according to the maker's specifications, enough air in the dress to keep a man alive for 15 minutes). On the way they rescued two fishermen clinging to a float. They arrived at night (after a helicopter had landed on the upturned hull and resuers had banged on the hull to try to locate survivors during daylight) and made an initial survey dive recovring one body caught in the rigging. The following day more bodies were recovered from the accommodation and more 5 from the bridge. Later, after autopsy, they were told that most of the dead recovered after the disaster had died from drowning from what they believed to be wind driven rain leading to the recommendation that rain b;ocking hoods be built into lifejackets. The surviving Indonesian diver's haliburton briefcase was recovered during the accommodation search. At the time of the incident, the AODC was debating the issue of a minimum of 2% O2 in the base gas, McDermott disagreed with it, one reason was "It will be difficult for the LST to calculate blow down", the job was an air sat weld at around 15 - 21 msw, the industry had just started using air sat's then, air being cheaper than Heliox, base gas was pure N2. The dive panel was an “in-house� built very small dive/sat panel. One of the chambers wasn't being used, so the take off was being used to flow gas to the welding habitat to keep the water out. During the dive (Air sat, so as usual at the time, the attitude of "Oh its only an air dive, don't need to analyze the divers gas", and the analyzer was either turned off or not even installed. One of the chambers had lost a few FSW, so the dive sup reached over, turned off the air supply to the sat panel which was supplying air to the habitat, turned on the N2, which was chamber make up, made up, and put air back on. Unbeknown to all at the time, the sat panel and dive panel had no check valves installed, N2 around 200 psi, air around 150 psi, (either way, the N2 to the sat panel was higher than the air to the dive panel), N2 tracked over to the dive panel, down to the diver, and zap, diver passed out from being given pure nitrogen. Initially not knowing what had happened, they were going to jump the bell man, (who was on same gas supply). Fortunately, before he donned his mask the bellman saw the diver underneath the bell and pulled him in, but he failed to respond to treatment. After the fatality, a SSOL safety rep was mobilized to the job in to carry out an investigation, (because of the 6 SSOL divers in sat). The panel designer/builder also arrived on site with a dive tech, SSOL told the dive tech to disconnect the line connecting both panels, and the crew to mix up the N2 base gas to around 9% to give a PPO2 of around 210 mbs at the working depth, (Think it was around 50 fsw). This was done, but as the investigation team was departing they noticed that the dive tech had connected the pipe up again, when he was asked why, he said "Well it hasn't happened before, and will probably not happen again.............." There were rumours that the dive supervisors were reluctant to sign off the dive logs and reports that showed the fatality was due to pure N2.

The bell was recovered to 150' and the surface (air) diver deployed to help. The bellman and surface standby diver managed to pull the diver into the bell and close the bottom door.   187. 2018 6 23 Firdaus Mohamed Singapore Marine Diving & Engineering Vopak Terminals SCUBA A 27-year-old commercial diver died after he was struck by a concrete pile while working in the waters off Pulau Sebarok on Saturday afternoon (Jun 23). Mohamed Firdaus was working as a commercial diver for Marine Diving & Engineering (MDE), which specialises in underwater services including marine and underwater construction. On the day of the accident, Firdaus was working on a project developed by Vopak Terminals Singapore on Pulau Sebarok’s Jetty 2. The project's occupier is HSL Constructor, which had engaged MDE for the project. "Two divers were in the water, installing brackets onto a concrete pile, when one of the workers was struck by a concrete pile," said the Ministry of Manpower. Firdaus was unconscious when taken to National University Hospital, where he died from his injuries, added the police, who received a call for assistance at 5.13pm. The police are investigating the unnatural death. Advertisement MOM has stopped all works at the worksite and is currently investigating the incident. In an email to Channel NewsAsia, Vopak Terminals confirmed that a fatal accident happened at its terminal in the port. "We are deeply saddened by this accident. We, jointly with our main contractor and his employer, are providing his family with all possible aid and assistance they need during this difficult time," said Vopak Terminals. Vopak Terminals also said it has started "a thorough investigation to find the root cause" of the accident. On Sunday afternoon, representatives from MDE, HSL and Vopak attended Firdaus' funeral, MDE and HSL said in a joint statement. "No words nor gesture can replace the loss of Firdaus, and we will certainly be here for his loved ones. We truly share the grief of his family, friends and colleagues," said Mr Masahiro Aoyagi, managing director of MDE. "This is the first fatal accident ever experienced by MDE since our incorporation 38 years ago and we are doing our utmost to understand what happened," he added.   193. 1976 4 16 Rig 'Ocean Express' USA Marathon Rig move from Mustang Island to a new location in Mustang Island East, 33 miles away towed by the Gulf Knight, Gulf Explorer and Gulf Viking. At 2300, the rig was about 1 mile from new location and jacking-down of the mat commenced but sea conditions deteriorated, with seas up to around 10 feet by 0600 on 15 April. Through the day, the seas continued to build with swells washing over the decks by afternoon and some water leaking into the accommodation. At 1510, one of the Gulf Knight's engines failed, followed by the parting of the Gulf Viking's towline at 1930. Numerous attempts were made to reattach the Gulf Viking's towline but heavy seas breaking over the deck frustrated the crews efforts. The water also shifted some of the drillpipe stored in racks on the deck, which the crew tried to resecure. As a result of the water washing over the deck and the loose pipe, work on the towline and pipe was abandoned as conditions became too hazardous. Around 2000, a Coast Guard helicopter was despatched to evacuate the crew. At 2115, the derrick shifted to starboard giving the rig an immediate starboard list of up to 25 degrees. The crew then abandoned the rig using survival capsules #1 and #3, sailing into 25 foot seas with 65 mph winds. The Barge Mover remained behind in an attempt to save the rig. However, when the Coast Guard chopper arrived at 2120, the Barge Mover requested evacuation from the heildeck then ordered the tugs to let go their towlines. The Coast Guard chopper then made two abortive approach attempts, during which time the rig's list increased to an estimated 45 degrees. The pilot's complete lack of visual reference in the night while attempting to hover over the rig rendered the extraction almost impossible but on the chopper's third approach, the winchman managed to lower the passenger basket just in time to scoop up the Barge Mover. The pilot said later that it appeared at this point that the chopper was rapidly losing altitude. The truth was that the rig was actually capsizing and the bow helideck was swinging up underneath the chopper. Seconds after the Barge Mover was picked up, the rig capsized to starboard. The 14 crew in capsule #1 were rescued by the Nicole Martin survey vessel, whose captain maneuvered the capsule into the lee of the vessel, allowing the crew to jump from the capsule to the Nicole Martin. Capsule #3 was not so lucky. After coming alongside the Gulf Viking, the capsule was flipped unexpectedly by the high seas and rapidly half-filled with water, preventing the capsule from self-righting. An air pocket formed inside the capsule, but after around 30 minutes only 7 men had managed to escape from it. The other 13 crew were found drowned the next day still inside the capsule. Loss of directional control, resulting from the towline breaking and engine failure, was considered the primary cause of the capsize. One of the Marine Investigation Board's findings was that 'allowing a low freeboard self-elevating drilling unit to drift broadside to boarding seas... is an invitation for loss'. The report also noted that the possibility existed that grounding of the rig may have contributed to the capsize. Both the increased weight from boarding seas and the angle of the starboard list allowing the mat to touch bottom may well have resulted in a 'tripping action' which increased the rig's heel to starboard. Jacking operations ceased and repair operations commenced, where a chock block was inserted to prevent movement of the jacking cogs and the defective motor was removed. Whilst easing the weight onto the chock block, gears on the number 1 jacking motor starting turning causing the starboard side of the barge to lower. The aft starboard leg brakes then failed causing the rig to fall onto its starboard side. Several men, including the crane operator, were injured during the collapse as personnel were being transferred onto the rig from a crew boat by crane. Most of the forty-one crew were picked up by nearby vessels, with two crew airlifted by the Coast Guard to a Gulfport Hospital.

The equipment was not in good condition, and both the primary and alternate regulator were leaking and in need of repair. Shortly after he submerged, the tether line floated to the surface. After he was signaled without response, the team leader put on scuba gear, submerged, and found the diver on the sea floor with a weight belt on and both tether line and tank high-pressure hose severed. The diver was recovered, and CPR was unsuccessful. The investigation did not determine how the hose was severed, and the cause of death was listed as drowning. OSHA cited the employer for violations including inadequate training in using tools/equipment and in CPR, absence of a ready standby diver, diver not line tended, lack of a reserve tank, and rescue not conducted in a timely manner. On Tuesday he and the first diver, an Indonesian named Bernardus Senge, were diving for pearlshell from the lugger ‘Bin Tang Putteh’ off Bard Creek in 13 fathoms of water. Albert had previously worked in only five to six fathoms. At the conclusion of the day he came up, apparently suffering no ill effects, but after midnight it was found that he had diver's paralysis. He was dressed in a diving suit and lowered into the water to the depth he had been working, where he went through the treatment of staging. When the vessel was ordered to return to shore because of inclement weather, he, along with his co-workers, were taken to the home of a Louisiana oilfield diver's supervisor where they were to remain on call and "on the clock" for further instructions'. The lawsuit claims that the diver's supervisors served their employees alcoholic beverages while waiting to return offshore. A supervisor allegedly supplied him with the keys to a motorcycle despite knowing that he had been drinking heavily. The defendant is accused of negligence for failing to properly plan for the evacuation, bunking and quartering the crew, providing alcoholic beverages to its crew, allowing and providing him with the keys to a motorcycle knowing that he had been drinking and failing to provide him with a safe and nonhazardous workplace�

  217. 1961 6 2 Ward James UK Kirkaldy Corporation 6 S/S Air Diving from a tug near the dock gates in Kirkaldy Harbour, alarm raised when tugman could not get response on lifeline. A team of divers from HMS Safeguard, Rosyth, raced to the harbour and located the diver trapped by his left arm under the sluice gates and by water pressure. In addition to his lifeline, a further two ropes were tied to him and the sluice gates lifted until he could be pulled clear. On deck, he was cut out of his suit and a doctor attepted CPR, but he failed to respond. Evening Times. His widow was awarded £3,487 and 10 shillings. The jury assessed the damages at £4,650 but held that the deceased was 25% to blame for the accident that led to his death. Unknown to the diver the sluice gates had been left partially open. As soon as he approached the bottom of the gate he was caught in the current of water passing through thr sluice and asphyxiatyed. Two employees monitoring the horse said that about 22 minutes into the treatment, he became agitated and started to kick (conflicting statements as to whether the horse was sedated on this occasion, though it had received 4 or 5 treatments before without incident), they tried to activate an emergency decompression sequence but the horse had already kicked off a quarter inch thick protective coating. They saw a "massive spark inside the chamber and then flames", Ms Moneley left the panel to call the fire department, Ms Marshall began the decompression sequence. There was then an initial explosion followed by a much larger blast that blew debris 1,200 feet, ripped off part of the roof and the side walls. Eleazar Viernes, 63, a diver from Olongapo City, died while working underwater with another worker to install a dummy gate for replacement of valve of its Pacific Dock. Viernes, however, was reported by another worker to have suffered difficulty in breathing underwater. He died an hour later at the hospital.

Called in to work on a broken dock gate wire Reported as drowned due to an acute lack of oxygen (One of the three three cylinders of compressed air issued to him was a nirtogen cylinder). They seem to have been using surface cylinders/umbilical similar to an SRP set up but without panel/back-up gas. The diver changed cylinders and returned to the water after a tea break but after a few minutes topsides realised there were no bubbles coming up, tugged on his lifeline, no response. Diver pulled to the surface, unconscious, transferred to hospital, declared dead. At the inquest, Singapore Oxygen Air Limited (SOXAL) stated they had supplied 25 cylinders of compressed air for industrial use (Light grey) and one cylinder of Nitrogen (Light grey with a black top) a few months earlier and that each cylinder also had a label stating the contents attached before they left the factory. They also added that cylinders with light grey bodies and black and white quarters at the top contained air for medical uses. The storeman who issued the tanks on the day had not noticed any gas contents labels on the cylinders but recalled that the cylinders were gray and at least one had black and white quarters. Additionally he said that he was not aware of which gases were in cylinders with different markings nor what colour a nitrogen cylinder should be as he was not in charge of sorting them. Investigators did not find any label on the Nitogen cylinder used by the diver. The Coroner recorded an open verdict and criticised the parties involved for their apparent lack of safety consciousness. Straits Times

Was distressed during decompression and after treated with a therapeutic re-compression but died in the chamber. Diving supervisor initially suspected pneumothorax but was over-ridden by the doctor who diagnosed the symptoms as pneumonia (The doctor involved was inexperienced in hyperbaric medicine).

A body has now been found with the life jacket uninflated. The recovery crew pulled the jacket inflation on recovering the body and it functioned correctly. Additional, but as yet unconfirmed reports, indicated that Kazair services employed three divers from a local diving contractor who were deployed from the helicopter, that a vessel in the area decided it was a good opportunity to launch its FRC during the exercise.(Not planned) and it was that FRC which recovered the second diver (it is assumed the first diver was recovered by helicopter), the third diver disappeared under the water and his helicopter lifejacket failed to inflate. Sequence appears to have been:- Reported feeling funny, was asked to flush hat from bail out, no response, Supervisor switched him to HP supply, pulled back to cage, deck, hat off, O2 administered, came round. From going on to HP to hat off on deck, 3 minutes. HSE investigation. Root cause appears to have been foul road compressor air from air lance buffeting it's way upwards into the helmet past a loose neck dam. Possible additional seabed contamination from Methane and H2S. (NB Road compressor was sited well clear of diving compressors, did not contaminate diving gas, contamination took place at the work site).   229. 2012 3 31 Kay Marson Ashly USA Karst Underwater Research 54 SCUBA Aged 29, volunteer diver, one of a six man team from Karst Underwater Research (A not-for-profit organisation that maps and measures flow and water quality of underwater springs for state agencies and water management districts in Florida), diving with three others at the Weeki Wachee Springs State Park. The group had descended to about 180' and as they ascended he took a different route and became wedged, possibly trapped by water flow (at about 100' depth), the team members tried but failed to release him. Rescue divers got to him in under three minutes from leaving the surface when the first group surfaced and raised the alarm, but found him dead with his regulator out and mask pushed onto his forehead. The autopsy concluded death was caused by an air embolism (specifically, that an air bubble had lodged in his heart and blocked the blood supply to the lungs) and the medical examiner ruled it as an accidental death. Karst U/W research quoted as saying "Instead of following the ropes as he had done multiple times in the past, Marson rapidly moved into a restrictive area of the crevasse. It is believed that this behaviour was not calculated but a reaction caused by the affect of an air embolism he incurred while rapidly ascending from depth. Typically, this condition causes profound changes in mental functioning, including disoriientation, blindness, paralysis, seizures and loss of consciousness within minutes or even seconds of onset. If it occurs after surfacing, it is often fatal or profoundly disabling even with prompt recompression therapy. When it occurs underwater, the incapacity or unconsciousness it causes almost always results in drowning". Hernando Today. This fatality has not been included as a commercial diving fatality as this seems to be a weekend cave diving group using 'research' as a means of gaining access to cave systems that might otherwise be off-limits (TC).

Master Divers, Chapel Street, Liverpool 4 S/S Air Aged 53, experienced diver, initially reported as 'Met his death whilst at wotk'. Hornby dock, Liverpool, working on the vessel 'Gulf of Taranto'. He was placing a sealing pad over a valve inlet diffuser on a vessel hull, differential pressure incident (arm pulled into hull when the valve was removed because the diffuser was mounted on the valve not the hull). They managed to pull him free but he was unconscious when recovered to deck, taken to Bootle hospital, but failed to respond to treatment. Coroner’s verdict:- Suffocated. Master Divers, Chapel Street, Liverpool 4 S/S Air William Storey, the incident:- Liverpool diver William STOREY, of 8 Toxteth St, a man of middle age and great experience, was engaged in the pursuance of his risky occupation when he met with his death under strange and peculiar circumstances. STOREY was working with two other men at the steamship ‘Gulf of Taranto’ lying in the Hornby Dock. The valve in the hull of the vessel required repair, and to enable this work to be carried out it was necessary that a "pad" should be fixed on the hull, so that the valve might be removed for repair without risking the flooding of the ship. This is the work that STOREY had to accomplish. A platform was lowered alongside the vessel, and STOREY, properly equipped in his diving-suit, went below. He signalled to the man in charge of the life-line to lower the stage. The next signal called for the "pad" A few minutes after the "pad" was lowered, yopsides received the signal "All right" which meant that the valve could be removed any moment. On board the vessel was the superintendent engineer of the Gulf Line of steamers, and as soon as the word was received from the divers the valve was unscrewed he called attention to the fact that the "pad" was leaky. Water was spurting in then the next moment a portion of the pad and the diver's arm were forced through the aperture. About this moment STOREY signalled to be drawn up, but the pressure of the water increased tremendously by the suction through the now open valve, jammed him tight against the vessel, and hauling him to the surface proved a matter of great difficulty. Mr M'KAY forced the diver's arm out through the aperture again, but the combined efforts of three men, instead of the customary one were necessary before the poor fellow was got out of the water. Storey was then unconscious and apparently dead, the sleeve of his diving-dress considerably torn, having been damaged by the violence with which his arm had been forced through the valve hole. When his headgear was removed it was found that he had been bleeding badly from the ears. The Bootle Fire Brigade horse ambulance was promptly summoned and STOREY was taken to Bootle Hospital. Upon arrival he was examined by the house-surgeon, who pronounced life extinct. Appearances hardly point to drowning and it seems more probable the unfortunate diver was either suffocated when his dress was torn open at the sleeve, or that the life was crushed out of him against the side of the vessel when the valve was removed without the "pad" apparently being properly secured

Master Divers, Chapel Street, Liverpool 4 S/S Air er was fast in some way, he called the man tending the air pipe even with his help it was impossible to raise the diver. He asked for assistance from a flatman with the combined efforts of the three of them they managed to raise the diver. Witness rapidly took off the diver's mouthpiece and saw STOREY was apparently dead. The deceased was at about 12ft under the water and from the time the signal came to pulling him up only minutes had elapsed. The Coroner elicited the information that the ship's hull was covered with a perforated metal rose, after the style of a rose on a watering can. Witness said, this rose was normally affixed to the skin of the vessel but in the present case it was affixed to the valve, this was important and the diver ought to have been told of it. Had witness known of it he would have used a wooden plug to stop up the hole instead of a pad to cover it. The hole was six inches in diameter. The post mortem results were that the skin of the deceased was a dusky colour. Blood had oozed from the right ear and there was a large bruise on the upper part of the left forearm and elbow. There were no broken bones or injuries that might have caused death. The vessels of the brain were congested but the brain tissue healthy. The lungs were very much congested and there was a good deal of mucus in the bronchial tubes. The small air vessels of the lungs were ruptured by the efforts to inspire. The heart was flabby and rather fatty, the valves being more or less closed, especially the mitrial and aortic. The blood throughout the body was a dark colour, a marked indication of suffocation. The cause of death was suffocation. The Coroner then addressed the jury, summing up the evidence and pointing out it was for them to decide whether any one was criminally liable for the death of the diver, they could express an opinion as to what led up to the suffocation which according to the medical evidence was the direct cause of death, or they could leave that point alone. After deliberation in private the jury found that death was due to suffocation, but how that was brought about they were unable to say. The funeral was reported as:- The remains of William STOREY, Diver, were interred yesterday morning in Toxteth Park Cemetery in the presence of a large number of friends. The deceased who was an experienced diver unfortunately met his death last Wednesday while working on the steamship Gulf of Taranto in the Hornby Dock. The coffin was covered with beautiful wreaths and the brass shield bore the inscription, "William STOREY, died September 30. 1896, aged 53 years�

Paraphrased from initial press reports:- “State police have recovered the body of a SCUBA diver who disappeared while looking for some missing construction equipment in a Monmouth County river. Robert Renner was part of a (Dock builder) crew that's constructing a 65-foot fixed span bridge to replace the aging Highlands-Sea Bright Bridge, but was off duty when he entered the Shrewsbury River on Saturday. He was trying to recover a jack that had fallen from the bridge earlier in the week. Diving solo from a 20' flat bottomed boat, went in with a downline tied to his arm. The rope came loose and he failed to resurface, the two people who were with him in a boat notified authorities shortly before noon. His body was recovered about six hours later. Officials say the area where Renner was diving is filled with debris and known for swift currents, and rescuers had to wait a few hours until they could safely enter the water. His badly decomposed body, missing off Western Singapore for two days, bobbed to the surface on Sunday evening; air tank taken by police for probe. In between running his own scuba diving firm, known as Jet Scuba, Mr Chia did freelance work for commercial diving companies. Relatives identified him through the tattoos on his body. The body of Mr Gerald Chia Jia Jie, 21, was found near the oil rig he had been working on, still clad in dark-blue work overalls and with his gas tank and face mask still in place. A Maritime and Port Authority of Singapore (MPA) boat, part of the search operations that had been under way for more than 50 hours, spotted the body at about 6pm. When Mr Chia went missing at around noon last Friday, he was on only his second assignment underwater for J Diving Services. He had started freelancing for the company only two days before that. He was carrying out installation works on the oil rig berthed in the Jurong West Anchorage at the time, 'his job was to install shackles on the rig, a relatively simple task', said the diving supervisor. 'Seasoned divers would take 15 minutes to do it, and newcomers, half an hour at the most', he added. Mr Chia had gone underwater, about 20m down, with a more experienced partner, as is the practice in the industry. Diving contractor was fined £1,200 after some kind of legal deal. Allegations during the inquiry that he committede suicide by cutting his umbilical, Sherriff determined it was accidental death with the most probable cause being that he got into difficulties and was forced to cut his umbilical and ditch his helmet. Diving conditions in the rough shallow water were described as 'hazardous'. Criticism by the Sherriff for it only a 3 man team (the diver, a supervisor who doubled as stand-by diver and an unqualified tender), lack of involvement/supervision by Customs and Excise, lack of communication with a police diving team on site. Widow intended to sue (defunct) diving contractor and Customs and Excise. No further details. Reported in the Herald Scotland

After 35 minutes he came to the surface by himself was hauled onboard but found to be dead.
